This volume finally sees Tetsuo come to terms with what he has become, as well as discovering how he became like that in the first place, as he fights the demons in order to stay this side of human. Kaneda must choose whether to kill his old friend and say humanity of try to save what is left of him.

For those who were first attracted to the legend of Akira through the Manga movie you will realise now how that movie only scratched the surface of what is a much more complex and thought provoking work.
